Providing effective health education for patients with diabetic retinopathy (DR) is a key strategy in health management. This study, based on the Knowledge-to-Action (KTA) model and clinical context, applies the Promoting Patient-Centered Learning (2012 Edition) guidelines to DR patients, emphasizing the central role of patients in the learning process and promoting health management.
Background: Providing effective health education for patients with diabetic retinopathy (DR) is a key strategy in health management. This study, based on the Knowledge-to-Action (KTA) model and clinical context, applies the Promoting Patient-Centered Learning (2012 Edition) guidelines to DR patients, emphasizing the central role of patients in the learning process and promoting health management. Methods: The multidisciplinary collaborative intervention program is constructed based on the LEARNS model. By using standardized assessment tools, the patients' learning abilities and cognitive baselines are quantified. A cross-professional education team is formed to develop personalized learning pathways, with a dynamic adjustment mechanism implemented. Monitor the differences in indicators such as patients' mastery of health education knowledge and nurses' reflection and insights.Results: The study systematically integrated 10 evidence-based recommendations and established 6 quality control review indicators. After the intervention, the total score for health education knowledge in the observation group of patients increased from (59.5±4.02) to (80.75±5.52), with a 15% improvement in nurses' reflection and insights. The compliance rate for all review indicators reached 100%, and a standardized educational pathway for DR patients was established. Conclusion: This study established a multidisciplinary collaboration mechanism and implemented a patient-centered personalized learning plan, which simultaneously enhanced nursing evidence-based decision-making capabilities. The resulting standardized educational pathway provides a replicable and highly effective evidence-based practice model for the health management of DR patients.
