Summary

Through the implementation of best practice guidelines such as Person and Family Centred Care, Preventing Falls and Reducing Injury from Falls, Alternatives to Restraints, Assessment and Management of Pressure Injuries for the Interprofessional Team and Transitions in Care and Services, Hampton Terrace Care Centre has made impactful changes in our home.  These changes work to enhance the quality of life of our residents, while helping to empower our staff.

Abstract

Our Journey to Enhancing Resident’s Lives through Evidence Based Practice

Hampton Terrace Care Centre began a journey in 2021 to enrich the practice of care in our home through the implementation of best practice guidelines.  We continue to implement new interventions based on evidenced based practice to improve the care our residents receive.

By implementing the Person and Family Centred Care, Preventing Falls and Reducing Injury from Falls, Alternatives to Restraints, Assessment and Management of Pressure Injuries for the Interprofessional Team and Transitions in Care and Services best practice guidelines, we have made impactful changes in our home.  Changes that work to enhance a residents quality of life while also empowering our staff.

Solid outcomes influenced by the utilization of evidenced based practice has included: 100% screening for delirium on admission, more thorough history taking, a more resident centered approach such as recognizing indigenous people and their beliefs on admission, preferred names, gender identity, sexual orientation, admission information sheet to communicate to front line staff a more holistic picture of who our residents are and improved identification and assessments of pressure injuries.

The implementation of best practice guidelines in our home has positively impacted the culture of care at Hampton Terrace.  We continue to work towards creating an environment of evidence informed care which includes effective utilization of information gathering prior to admission and on admission, showcasing our residents as the whole person that they are, screening for delirium and pressure injuries on admission and working to promote an open dialogue with staff on how best we can improve resident outcomes. At Hampton Terrace, our mission is to promote and enhance resident-focused care and quality of life through excellence, teamwork, integrity and innovation and our work as a Best Practice Spotlight Organization helps us do that.
